The Veteran’s Application Form

If you are a VA Rated Disabled Veteran from any conflict living in the Alamo Region area, you are eligible to apply for a home adaptation through Adapt A Vet.

Complete the form online to the right or download it and return it to Adapt A Vet at [email protected].

In addition to filling out the Veteran’s Application Form, the following items must be emailed to [email protected]:  1) a copy of your DD 214 (Certificate of Release or Discharge), 2) VA disability rating determination letter, and 3) a recent photo.
